ABOUT US

No matter what your age, Southwest Center on Aging is where you'll find the advice, information, and support you need to begin maximizing your health and well-being.

I don't subscribe to the concept of "anti-aging" or the view that we can reverse the physical changes that come with growing older. The process of aging is a biological fact, and you can't turn back the clock.
However, I do believe we can age with grace, and that we should do everything in our power to delay the onset of age-related disease, discomfort and loss of vigor.

Southwest Center on Aging will help you develop the lifestyle practices essential to experience optimal health, physically, mentally, and spiritually.

Dear Patient,

​Welcome to the Patient-Centered Medical Home (PCMH) initiative, a new way of managing your health care! PCMH is a model of care designed to improve the coordination of your health care with an emphasis on your all-around well-being.

I invite you to continue working with me in this new model of care. I will work with other health care providers to take care of you. As your care team, we will involve you in decisions about your health and health care, and thus be able to develop a stronger relationship with you.

The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others. It was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts. The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.

The practice is concerned about the range of the patient’s whole health, including behavioral health. We are responsible for coordinating care across the healthcare setting.

WHAT WE OFFER

Southwest Center on Aging offers a unique, comprehensive assessment of older persons in an outpatient setting. We use multiple resources to look at the individual from medical, functional, and emotional perspectives. Our goal is to work with the patient’s family to address strengths and weaknesses found during the assessment process. This assessment is valuable on a consultation basis or as a first step to ongoing primary care with us.
  • We provide Primary Care to seniors in a friendly office setting. 
  • We coordinate medical, social, and hospice services for patients and families facing terminal illnesses. Assistance is provided in establishing Advance Directives, selecting resuscitation status and designing a Durable Power of Attorney.
