OUR SERVICES

We are committed to improving the health of older persons through preventive health services, comprehensive assessments and long-term care. Patients come to us for Dr. Duran's expertise in aging, reputation for compassionate, unhurried care, his patient advocacy and inclusion of family members as vital participants in the treatment plan.

We welcome all patients seeking a doctor specializing in internal medicine and/or geriatrics - from younger, fairly healthy people to those with more complex needs such as memory loss, depression, functional decline, social issues and chronic medical conditions. All patients share a common desire to be listened to and cared for, and Dr. Duran is dedicated to meeting those needs. Our patients are active members of their healthcare team, working with him to develop personalized healthcare plans that address their current and long-term needs.

​COMPREHENSIVE GERIATRIC ASSESSMENT

We offer a unique, comprehensive assessment of older persons in an outpatient setting. Dr. Duran uses multiple resources to look at the individual from medical, social, functional, and emotional perspectives. Working with patients, families, and community supports, a plan is suggested to address strengths and weaknesses found during the assessment process.
This service is available to all elderly persons but may be especially valuable to those whose physical, functional, or emotional status has become unstable. This assessment is valuable on a consultation basis or as a first step to ongoing primary care with us.

​OUTPATIENT GERIATRIC MEDICINE CLINIC

We provide outpatient medical services in an office-based setting. Our office is located in Southwest Cardiovascular Center just minutes from both hospitals. Special emphasis is placed in prevention and healthy aging, as on screening for dementia and depression, assessing functional abilities, and identifying risks for decline in health status.

TRANSITIONAL CARE

Southwest Center on Aging created the Transitional Care Program to enhance care for chronically ill patients after they leave the hospital or Skilled Nursing Facility.  The program is focused on making sure your transition from the hospital or Skilled Nursing Facility to home is smooth and that all your follow-up care needs are addressed. Each patient is contacted within 24 hours of discharge by the transitional care coordinator and a provider will be seeing you in the first 72hrs of discharge. The amount of contact after that is individualized based on patient need.

​LONG-TERM CARE

Dr. Duran treats residents in a variety of long-term care settings. Patients needing skilled nursing and therapy services after hospitalization are cared at community-based skilled nursing facilities. Primary and specialty geriatric care is delivered on-site to residents in several local nursing homes and Alzheimer's care facilities. Selected Personal Care Homes and Assisted Living Facilities are additional sites where home-bound and frail elderly residents are routinely seen by Dr. Duran. In these settings, the focus is on preservation of function and maintaining quality of life when addressing the medical issues.

HOSPITAL CARE

We deliver the primary medical care for established patients that need hospitalization or evaluation at the hospital. Dr. Duran routinely provides consultative services on our patients who are admitted to the hospital for surgical or other specialty services. Inpatient geriatric consultations are performed, at the request of other physicians, for any other patient who may have multiple medical conditions, dementia, functional impairments, or wound care needs. Dr. Duran frequently assists other physicians with complicated placement and psychological concerns, palliative care needs and end-of-life issues.

HOSPICE CARE: COMFORTING THE ILL AND PALLIATIVE & END-OF-LIFE CARE

Working as a team, Dr. Duran coordinates medical, social and hospice services for patients and families facing terminal illnesses. Pain management and control of other symptoms is a priority in ensuring optimal patient comfort. Assistance is provided in establishing Advanced Directives, selecting resuscitation status and designing a Power of Attorney. Care plans are individualized to the need of the patient and family, and focus on maximizing quality of life and comfort.

DEMENTIA AND MEMORY LOSS

​We provide appropriate therapy in individual, family, and group settings. In addition, the Geriatric Mental Health Clinics work collaboratively with experts from Geriatric physicians to provide ongoing care for individuals with dementia and their families.​

​HOUSE CALLS

House calls are offered to those patients who are no longer able to seek medical care in the office or clinic settings because of physical or emotional limitations. House calls may be used temporally, until someone is again able to return to the outpatient clinic, or it may be used as a permanent way to receive health care.
